Meeting the health care needs of the communities we serve is the cornerstone of California Pacific Medical Center’s not-for-profit mission. That mission extends to serving those who cannot afford health care and to supporting programs and services for those in financial need. The California Pacific Medical Center Community Health Programs Department develops community benefit programs and services to improve health in San Francisco communities and increase access to health care.
The Community Health Programs Department vision is to reduce health disparities in vulnerable populations. We enhance the quality of life of all people we serve by promoting their health and well-being. We work collaboratively to build capacity within the communities and ensure equal access to culturally and linguistically appropriate services and resources
Our mission is to transform the lives of the people in our community. Our efforts focus on actively engaging community members and stakeholders in the planning, designing and delivery of holistic, compassionate and caring services and programs that address unmet needs.
2008 Community Health Grants Program
As a not-for-profit committed to meeting the health care needs of the communities we serve, California Pacific Medical Center proudly develops and supports programs and services for those in financial need. Our goal is to improve health in San Francisco communities, increase access to health care, and reduce health disparities in vulnerable populations. We work collaboratively to build capacity within the communities and ensure equal access to culturally and linguistically appropriate services and resources.
As part of this important mission, we provide crucial funding through annual Community Health Awards, Investment Awards and Partnership Grants. This year the medical center distributed more than $750,000 to organizations that meet the diverse needs of our community. Grants ranging from $5,000 to $100,000 were awarded to 25 programs that are addressing care for: cancer patients, seniors, veterans, the homeless, low-income residents, immigrants, the LGBT community, and victims of rape, domestic and community violence. Funding has also been awarded to groups encouraging healthy lifestyle curriculum in schools, such as fitness, nutrition and weight management, as well as those focusing on education and prevention of HIV, hepatitis and chronic diseases.
